The metaphor refers to the sojourn in the desert but without discounting the interventions God made in the life of his people while they were in Canaan. "Healer" is the name of Yahweh in the Desert, summarizing the acts by which He saved Israel from death due to lack of food and water.

He is told that what he believes in is no longer valid: to be born an Israelite is not enough for the Kingdom of God; one must be born from above, i.e. in water and Spirit.
